Document Checklist
Please ensure all documents listed below are included with your completed application form. Incomplete applications cannot be processed. Certified copies are required — originals must be presented at the time of registration.
- Certified copy of the learner's birth certificate (unabridged)
- Previous school reports — most recent two years
- Transfer certificate from the current school (original)
- Immunisation / Road to Health card (certified copy)
- Certified copy of the parent or guardian's ID document
- Proof of residence not older than three months (e.g. utility bill, bank statement, or lease agreement)
- One recent passport-size photograph of the learner
Note: Originals of all documents must be brought to the registration session for verification. All copies submitted must be certified by a Commissioner of Oaths.
